Program Overview

NLF Premium range is the Class minimum and up.

Limitations:

• New ventures will be written, but they must generate a minimum of $5000 in premium
• NLF will not write accounts affiliated with Cypress, a related company
• Other restrictions may apply depending upon class code
• The minimum premium threshold for mid-term moves is $50,000, and NLF will not allow requests for effective dates within 60 days (before or after) the normal anniversary date

Requirements

• Completed and signed Acord application with FEIN and full description of applicant's business and operation
• Completed supplemental questionnaire
• Four years of payroll and premium history (by year)
• Four years of current valued carrier loss runs (dated within 90 days of effective date)
• Details of any claims over $20,000, with a description of insured's actions to avoid/reduce future losses of a similar nature
• Copy of the current Ex Mod worksheet for the applicant
• Bureau Inspection Report if available
• Most current financial statements for accounts over $250,000 in premium
