Requirements

• Completed Acord Application
• Complete COPE Information on Acord Application
• Four years current valued loss runs (valued within 90 days
• Building age and type and date of renovation for buildings built prior to 1980
• Supplemental application(s) and additional information will be requested some business classes, reflecting risk
